I don't know why, but for some reason trepanning has always fascinated me. You're right that the original caption on the Shorpy photograph is patronizing, not least because so many of those ancient Peruvians survived the procedure long enough for their skulls to partially heal, a not insubstantial accomplishment. Given the medical ignorance of the era in which the caption was written, it's almost ironic.

Re: the galactic core image: The way the fish-eye lens distorts the image, it looks as though the galaxy is undulating across the sky as though it were a live, tentacled thing. Wonderful stuff!
